Patients who wish to register with the practice need to provide us with their signed medical card which formally indicates that they wish to join this practice.  Our GP's will also sign the card to acknowledge that they are happy to accept the patient onto the practice list. Any patient who cannot find their medical card will be issued with a form to request this.  Completion of this form and a copy of the requesting patients photo ID and proof of address will be required. The Practice will then provide the information to the Business Service Organisation who will accept/ reject the application and to trace previous medical records. Patients applying to register who are not UK residents or patients who are returning to live in the UK will need to complete an HS22X form which will require more detailed information in order to complete the registration process.  It is important that you provide a list of your current repeat medication from your previous GP when returning your completed registration forms.

The Business Service Organisation has provided us with clear instructions for registering patients. Once we are in receipt of signed documentation from new patients, we will create a patient account on the computer and send an electronic application to the Business Service Organisation. They generally respond with 15 working days changing the "applied" status of the registration to "permanent".  If you need to see a doctor urgently or have run out of repeat medication please ask our reception team for further advice

Please note it can take up to 6 weeks before we receive previous medical records.
